Contents
Per puff of Berodual MDI Ipratropium Br 0.02 mg, fenoterol HBr 0.05 mg. Per mL Berodual soln Ipratropium Br 0.25 mg, fenoterol HBr 0.5 mg. Per Berodual Forte unit dose vial (UDV) Ipratropium Br 0.5 mg, fenoterol HBr 1.25 mg
Indications/Uses
To prevent & treat symptoms of COPD w/ reversible bronchospasm eg, bronchial asthma & especially chronic bronchitis w/ or w/o emphysema. Anti-inflammatory therapy may be used concomitantly in patients w/ bronchial asthma & steroid responsive COPD.
Dosage/Direction for Use
Berodual MDI Adult & childn >6 yr Acute asthma episodes 2 actuations for prompt symptom relief. In more severe cases, if breathing has not noticeably improved after 5 min, 2 further actuations may be taken. Intermittent & long-term treatment 1-2 puffs tid, up to a max 8 puffs/day. Berodual soln for inhalation Acute episodes of bronchospasm Adult & childn >12 yr 1-2.5 mL. Dose may be increased to 4 mL in severe cases. Childn 6-12 yr Acute asthma episodes 0.5-2 mL, depending on the severity. Childn <6 yr, <22 kg Ipratropium Br 25 mcg & fenoterol Br 50 mg per kg/dose, up to 0.5 mL (10 drops). Berodual Forte UDV Adult & childn >14 yr Treatment of attacks 1 vial. Severe cases: 2 vials.
Contraindications
Hypertrophic obstructive cardiomyopathy, tachyarrhythmia. Hypersensitivity to fenoterol or atropine-like substances.
Special Precautions
Underlying severe heart disease, recent MI, hypokalemia, insufficiency control DM, severe organic heart or vascular disorders, hyperthyroidism, phaeochromocytoma, prostatic hypertrophy or bladder-neck obstruction, patients receiving digoxin, MAOIs, TCA, patients predisposed to narrow-angle glaucoma, cystic fibrosis. Pregnancy & lactation. Berodual Forte: Not recommended to use in childn <14 yr.
Adverse Reactions
Fine tremor of skeletal muscles, nervousness, palpitations; less frequently tachycardia, dizziness or headache. Dry mouth, throat irritation or allergic reactions, cough, paradoxical bronchoconstriction (rare). Urinary retention may occur in particular, in patients w/ preexisting outflow tract obstruction.
Drug Interactions
Effects enhanced by other β-adrenergics, anticholinergics or xanthine derivatives. Glucocorticoids, steroids, diuretics, digoxin, β-blockers, MAOIs, TCA, halogenated hydrocarbon anesth.
